2.Procedures for reporting accidents using the electronic application system

The accident report to Yokohama City is "Yokohama City Electronic Application and Notification Service (hereinafter referred to as" Electronic Application System ") This will be done.
After 17:00 on March 25, 2022, the application destination has been changed to a new electronic application system. When applying using the new system, "advance business registration (login)" is required.

If the screen of the electronic application system is left open for a long time or the line is disconnected, the entered information may be lost. Please use the "Temporary Save Application" function as appropriate. In addition, items that are likely to be long, such as "The situation, background, response, etc. of the accident", "Cause of the accident", and "Efforts to prevent recurrence", should be compiled in a concise sentence in advance.

[Flow of Report]

  1. After the accident, each company immediately submits the first report to Yokohama City using the electronic application system.
  2. About progress report of accident processing, we report to Yokohama-shi as needed.
  3. Submit this report (final report) using the electronic application system where there is a break in the accident handling.
  4. Each business site actively discloses the contents of the accident report to users and issues it upon request.

3.Scope of Report

  1. Injuries of users (in principle, those that have been diagnosed by a physician and require some treatment such as medication) or fatal accident due to the provision of services
  2. Food poisoning, infectious diseases, outbreaks of tuberculosis
  3. Occurrence of violations of laws and regulations, scandals, etc. of employees (employees)
  4. Mistakes (including medication leaks and drops)
  5. Separation and missing
  6. Fire accident
  7. In cases where there is a risk of affecting the health condition of the user due to defective building equipment, etc.
  8. Those that the administrator deems necessary to report

4.Duty of explanation to users, etc.

Actively disclose the contents of the accident report to users and their families and issue it upon request. Also, please explain the following contents.

  1. Report the accident to the competent department.
  2. It may be reported to Kanagawa Prefecture as an accident case.
  3. When a request for information disclosure is issued, other than personal information may be disclosed.

5.Others

  1. The responsible section receiving the report will conduct surveys and guidance to businesses as necessary. .
  2. In addition, we may confirm the fact with the user or his / her family as necessary.
