For businesses that provide regular patrol / ad-hoc home-visit nursing, small-scale multifunctional home care, dementia-responsive cohabitation care, and nursing small-scale multifunctional home care, at least once a year, Based on "standards for personnel, facilities, and operation of community-based services", etc., evaluate the quality of nursing care services provided by themselves (self-evaluation), and evaluate the results from a third-party perspective at the Management Promotion Council etc. Is required. After the external evaluation, please report the result to the city.

Reducing the number of external evaluations

For establishments that meet certain requirements, the number of external evaluations can be reduced (once every two years). However, about self-evaluation (conducted for each unit) and making of target achievement plan (every establishment), carry out every year and submit to Yokohama-shi.
Please refer to "Office Administration Guidelines pertaining to the application of relaxation of the number of implementations" for requirements and procedures to receive the application of relaxation of the number of implementations of Yokohama City External Evaluation. In addition, evaluations utilizing the Management Promotion Council cannot be included in the years of continuous evaluation, which is a requirement to ease the number of evaluations conducted by outsiders.
