Those who are eligible to participate in this proposal are required to submit the proposal guidelines pertaining to "Advanced Care Planning Spread Awareness Drama Production Business Outsourcing".
All conditions described in “4 Conditions for Participation (1) or (2)” shall be satisfied, and the restrictions on “4 Conditions for Participation (3)” shall not be met.

1 Procedures for Participation
Businesses wishing to participate in this proposal should submit one copy of each of the following documents.
（１） Participation Application Form (Form 1) 1 copy
　※In the case of a joint proposal, submit one copy of the “Application Form for Participation Intention (Joint Proposal (JV)) (Form 2)”.
（２） Pledge (Form 3) 1 copy
　※In the case of joint proposals, submit one copy for each participating company.

2 Submission of Proposals
Please submit the following documents for those who have submitted the application form and who have been approved for the proposal qualification.
In addition, please make in predetermined style based on proposal preparation procedure [attached sheet 1] "about presentation of proposal".
（１） Proposal (Form 7) 1 copy
（２） 1 to 9 10 sets of proposal documents (paper output, double clipping each set)
（３） 1 copy of data (DVD-R) ※All data must be in PDF format.

Bringing or mailing
•If the submission deadline has passed, it will not be accepted. However, if the arrival is delayed due to the reason of the delivery company, we will accept it with proof.
•In the case of mailing, please write "Application Form / Pledge is present" and "Proposal is present" on envelopes etc. In addition, please be sure to contact the submission address by telephone or e-mail after shipping.
•If you bring your own, it will be accepted by the Medical Care Bureau Cancer and Disease Control Division from 9:00 am to noon and 1:00 pm to 5:00 pm on weekdays.

5. Answer Sending date and method We will send it by e-mail by Monday, March 22, 2021.

•We will confirm the proposal qualifications of the applicant who intends to participate in this proposal, and mail a “Proposal Qualification Confirmation Result Notice (Form 5)” regardless of whether or not they have qualifications. If you are able to confirm that you are eligible for the proposal, we will also send you an e-mail "Documents on the direction of drama production".
•Notification will be made by Monday, March 8, 2021.
•An applicant who has been notified that his / her eligibility has not been confirmed may request a written explanation of why his / her eligibility was not confirmed. In addition, please submit document to City of Yokohama, Medical Care Bureau cancer, disease measures section by 5:00 pm five days after city hall closing day from the next day when Motoichi sent notice.
•When an explanation is requested by the preceding paragraph, we will respond in writing to the person who requested the explanation within 5 days excluding the city hall closing date from the day after the city received the document.
