What should I do if the name or location of the establishment printed on the delivery form of the city / prefectural tax (special collection) changes?

If there is no change in the designated number, the delivery form can be used as it is.
Separately, please submit "Notification of location and name change of salary payer (special collection obligation person)" to Yokohama-shi special collection center.
