By informing you in advance in a health note that you would like to consider at the time of consultation, you will be able to enter the consultation smoothly.
２．When explaining medical history, life history, medication information, etc., you will be able to correctly communicate what you tend to forget.
３．By carrying this note in the event of a disaster, you will be able to understand the status of disability and medication, and take appropriate measures.
