Please cooperate with the 2025 demographic survey (occupation and industry)

　If you have child birth, died, stillbirth, or married or divorced, you must submit their respective notices at the ward office.
　The Ministry of Health, Labour and Welfare conducts a demographic survey based on these reports, and the results of the survey are used in various fields as information that contributes to the development of public health and the economy and society.
　In particular, in the year in which the census is conducted every five years, in order to conduct a demographic survey (occupation and industry), we request that each notification form be filled out with occupation (including industry in death registration/death notification form).
　Thank you for your cooperation when submitting these notifications in 2025.
　In addition, the information used in the demographic survey is strictly protected by the Statistical Law, so please fill in with confidence.

　※ death registration/death notification form should also include industries such as agriculture and forestry, construction, manufacturing, and medical and welfare.
　※ At the ward office window, there is a "Request for those who are notified of child birth / Death / Stillbirth / Marriage / Divorce (Occupation / Industry Example Table)" stating the classification.
