For those who apply for the payment of daily life tools (non-continuous items) with disabilities (children)
In Yokohama City, the “Yokohama City Severely Disabled Persons (Child) Daily Life Equipment Benefit Project” provides the necessary tools to ensure smooth daily life.
Tools to be paid vary depending on the type and grade of disability, target requirements, household status, etc. Please consult with Health and Welfare Center in your ward for requirements and procedures.
1. Necessary items for procedures
(1)Consultation and application before purchase are required. In addition, additional documents may be required in addition to the following.
(2)If a person other than the target person applies, please bring the identification documents of the applicant.

2 Estimate
(1)To create a quote, ask the company you plan to purchase after confirming whether or not this system can be used.
(2)The quotation must include the product name (product name, model number, standard, etc.), unit price, quantity, total amount, Address and name of the target person. Also, if you have any reference materials such as product catalogs, please attach them.
(3)As a general rule, products that meet the requirements of each item specified by Yokohama City are eligible for the system. The amount exceeding the standard amount will be borne by the user. For more information, please contact us.

3 User burden
(1)As a general rule, 10% of the cost required will be borne by the user. However, depending on the tax status of the household, the upper limit of the burden is set for each application.

(Note 1) The municipal tax amount is when the month of application is from July to March of the following year: For the relevant fiscal year, April to June: Check the previous year's one.
(Note 2) ①Regarding the municipal tax amount after 2018, even if you live in a designated city, you will continue to use the standard tax rate (6%) before the tax system revision. The amount of income percent is the amount before (2) special tax credits such as housing borrowings and (3) donation tax credits are deducted. In addition, we use calculation before 4th child, specific dependent relative deduction is abolished.
4 Number of benefits, useful life, and re-benefits
(1)As a general rule, the number of benefits can be paid is one per item. However, depending on the item, multiple payments may be possible only during the same year.
(2)The service life is set for each item.
(3)Repayment is not possible regardless of its useful life while the provided equipment can be used.
(4)If the equipment cannot be used before the end of its useful life and repairs can be made, repair it and use it. In that case, all expenses will be borne by the user.
(5)If repair is not possible, we will consult with you about re-benefit. In that case, in order to confirm that repairs are not possible, contact information of the business operator (stores, manufacturers, etc.) who purchased the equipment, documents created by the business operator etc. stating that it is impossible to repair, etc. May be brought.
5 Flow of Benefits

(2)In addition to the name of the item, the estimate of the first half (October to March of the following year) and the second half (April to September of the following year), the total amount, product name, and the target Address name are required.
(3)The total amount may exceed the standard amount depending on the required amount. The excess amount will be borne by the user.

4 User's burden
