3 What do I need for a tax payment certificate by mail?

(1) Application Form
Please provide applicant's Address name and date of birth, taxpayer's Address name and date of birth, tax type and required number of copies, purpose of use, and contact Phone number that can be contacted during the day.
(2) Reply envelope
Please put a stamp and write the address.
In order to protect personal information, it cannot be sent to anyone other than the taxpayer's Address.
However, only light motor vehicles tax (keijidosha-zei)'s tax certificate for vehicle inspection may be sent to Address in applicant.
(3) Fixed amount of money equivalent to commission amount (purchased at post office)
300 yen per case (per tax item per year). However, it is free for light motor vehicles tax (keijidosha-zei)'s tax certificate for vehicle inspection.
