1. Implementation period
   May-June 1995　
   From July to August 2015　
   September to November 31, 2015　
   From December 4th Reiwa 7 to February 198　

2. Targets
   Home care support establishments in Yokohama City (the target plan is 1-2 cases per establishment)

3. Details of implementation
   (１) Document check of care plan
   (２) Interview with the chief care manager of the Yokohama City care manager Council (about 60 minutes per care plan)
   　①Online (using Microsoft Teams)
   　②Face-to-face (Yokohama City Hall Conference Room)
   　　※Select either method
   (３) A report summarizing reflections (sent after interview)

4. How to apply
   Please apply for each establishment from the Yokohama City Electronic Application System.

5. Application period
   From April 1, 2015 (Tuesday) to April 30, 2017 (Wednesday)

   October 1, 2015 (Wednesday)-December 31, 2017 (Wednesday)

   ・In the case of a large number of applications, the deadline may be before the deadline.
   ・It is also possible to apply for establishments implemented in Reiwa 4-6, but depending on the application status, we may not be able to meet your request, such as giving priority to establishments that have not been implemented.
   ・Those who have taken the "Care Plan Writing Course that can be used from today" (held on March 17) will be given priority.
   ・This is not an operation guidance.
