1 Preparation of documents to be submitted and various reports

Form and spelling of necessary reports (buildings, building equipment, fire prevention equipment)

  • Please see this "Download Form List" for downloading periodic reports, summary books, survey results tables, etc.
  • Please refer to this "Regular Report required documents and How to Spell (PDF: 517KB)" for documents required for reporting. In addition, please bind the summary document (1 in PDF above) and the report (2-5) separately.
  • When submitting a report, you will need a summary document and one copy of each report. A receipt will be issued at the time of reception. In addition, if a copy of the reporter is required, if you can prepare one copy separately, we will stamp the reception stamp at the time of reception and return it. If you would like to return your receipt and copy by mail, please prepare a reply envelope with a stamp attached at the time of submission.

Notes on Report preparation (Building, Building Equipment, Fire Protection Equipment)

  • Please refer to "Entry of reports, summary documents, etc." in the "Download Form List" for notes and points to keep in mind when preparing each periodic report. In addition, if the documents are incomplete, we may not be able to accept them. In addition, if there are any deficiencies, we will respond by replacing them in principle.

※For items that have already been improved, please include the details in the report. Specifically, please see "about improvement contents and report entry method such as outer wall consultation (PDF: 19KB)".

※If "private room video shops, etc." is the target, please attach the "Survey Result Form (No. 4 style 2)".
※When "underground mall" is targeted, please attach "survey result vote (underground mall)" (Yokohama-shi notification No. 85 separate paragraph) in addition.

About various notifications

  • If a regular report is no longer required due to a change in use or removal, or if it becomes necessary again due to a change in use, a notification is required when the basic information such as the owner / manager is changed, or when the report time is changed. Please see "List of Download Forms" for the form of notification.
