(２) A person who has not received the nomination suspension measures based on the City of Yokohama, Medical Care Bureau Hospital Administration Headquarters Nomination Suspension Measures Guidelines on any day from the date of submission of the application form to the date of opening the bid.
(３) Being a person who has been registered in "insurance" in the list of qualified general competitive bidding qualifications (related to goods, consignment, etc.) in Yokohama City in Reiwamoto.
(４) Location classification must be "city" or "quasi-city".
