（1） You must not be a person listed in Article 3, Paragraph 1 of the City of Yokohama, Medical Care Bureau Hospital Administration Headquarters Contract Regulations and be qualified as prescribed in Paragraph 2 of the same Article.
（2） A person who has not received the nomination suspension measures based on the City of Yokohama, Medical Care Bureau Hospital Administration Headquarters Nomination Suspension Measures Guidelines on any day between the deadline for submitting the application form to the opening date of the bid.
（3） In the 5th and 6th Yokohama City General Competitive Bidding Qualified Person List (related to goods, consignment, etc.), registration is permitted in "021: Pharmaceutical", and registration is permitted in the detailed "F: Medical Gas" Being a person.
（4） Being a city company or a semi-city company
（５） Have been notified of the high-pressure gas sales business under the High Pressure Gas Safety Act and have been approved by the pharmaceutical sales business (wholesale sales business) specified by the Act on Ensuring Quality, Effectiveness and Safety of Pharmaceuticals and Medical Devices.
（６） A person who has a track record of manufacturing or delivery of the goods, or a person who can deliver the goods.
