When we receive routine immunization in medical institution outside the city, "vaccination conduct request book" issued by the mayor of Yokohama is necessary. (In principle, the vaccination fee will be paid by yourself [paid])
Please apply for the issuance of the “Vaccination Implementation Request Form” at Health and Welfare Division Health Promotion Section, a ward with resident registration.
