If you receive routine vaccinations at a medical institution outside the city, such as returning home and giving birth, you will need to apply in advance.
The application procedure can be done online from the link below.
　Application (outside site) when we carry out routine immunization for child outside Yokohama with homecoming births
After applying, Hodogaya Ward Office Health and Welfare Division will call you if necessary.
Specifically, please see when we receive routine immunization in medical institution outside the city (internal site) or refer to Health Promotion Section (Phone: 045-334-6344).

・Those who have a resident registration in Hodogaya-ku, Yokohama City as of the date of vaccination and who fall under 1 or 2 below
①Those who receive routine vaccinations at medical institutions outside the city, who are returning home and giving birth outside the city (including accompanying brothers and sisters) or are hospitalized at a medical institution outside the city.
②Those who receive routine immunization outside the city for other reasons

※Vaccination costs outside the city are entirely borne by you, but in the case of 1, you can receive a refund (redemption payment) of the vaccination costs by receiving a “Vaccination Implementation Request Form” in advance. Information on how to refund will be mailed together with the “Vaccination Implementation Request Form”.

It takes about 2 to 3 weeks from the date of application to issue the implementation request form. Please apply with plenty of time.
