Promotion of comprehensive community care in Isogo Ward

In the area where the elderly are accustomed to living as much as possible, nursing care, medical care, nursing care prevention, life support, and housing are provided integrally in order to continue their own life until the end of life (specifically, junior high school) This is a comprehensive support and service provision system for each district).

In March 2022, we revised the Isogo Ward Action Guidelines for the Establishment of a Comprehensive Community Care System for Yokohama, formulated in March 2018.
Reflecting the progress of initiatives since the formulation of the ward action guidelines, incorporating specific measures to respond to new issues, etc., the name was changed to "Isogo Ward Action Plan for Construction of a Yokohama-type comprehensive community care system."

Based on Yokohama-shi version "action guideline for construction of Yokohama-type community-based comprehensive care system" formulated in March, 2017, we devise action guideline of 18 wards of Yokohama-shi.
In March 2018, Isogo Ward formulated the Isogo Ward Action Guidelines for the Establishment of a comprehensive community care system for Yokohama.
