Emergency Medical Information Kit

We will always be sick at home and call an ambulance. In such a case, be prepared regularly to provide the necessary information.
※Emergency personnel will utilize it according to the situation. It is not a system that promises to be used.
What is the Emergency Medical Information Kit?
In case of emergency, contact information of family acquaintances, medical information such as family doctor, chronic illness, medication, health insurance card (copy), patient's card Shinryo-ken (copy), personal photo, etc. are placed in a container and stored at home. This is an effort to prepare for emergency situations.

Q What kind of people do you get?
A We will distribute it to those who wish to be Isogo residents. Also, we will distribute it in Neighborhood Association Neighborhood Associations units.
Q How can I get it?
Isogo Ward Office, Health and Welfare Division.

Q What should I do if I get it?
A Work according to the procedure shown in the figure above (how to use the emergency medical information kit). If there is any change in the information in the kit, please change it to the new content yourself each time. The ward office will not ask you to update it.

Q Do I put the display sticker inside the entrance? Isn't it easier to understand if you put it on the outside?
A Depending on the municipalities, this initiative may be being promoted mainly by the elderly and people with disabilities, so putting this sticker on the outside of the entrance may lead to troubles such as malicious door-to-door sales. There is also a possibility. For such reasons, please put it on the inside of the entrance.
