Passport application form

The passport application guide and application form are distributed at the Public Relations Section (101 window) on the first floor of the Izumi Ward Office.
　
At present, there is a very shortage of applications and it is not possible to hand it over to everyone. The application form is scheduled to arrive in early June.
If you are in a hurry, please use the download application form of the Ministry of Foreign Affairs or come to the passport center window.
In addition, the style of the application has been changed from March 27, 2023. Please note that cannot use the application of the old style. For more information, please check the Kanagawa Prefecture website (outside site).

There are two types, one for 5-year passport and one for 10-year passport.

• Kanagawa Passport Center Headquarters (outside site)

Those who can apply for a passport in Kanagawa Prefecture are those who have registered as residents in the prefecture.

• Yokohama City Centerminami Passport Center

Those who can apply for a passport at the Yokohama City Centerminami Passport Center are registered as residents in the city.

※You cannot apply at the ward office.
※If you live in Yokohama City, you can use either Honjo (Yokohama) or Centerminami.
※If it is deemed appropriate from the viewpoint of the convenience of the applicant, such as commuting to school or commuting, you can also use the passport center branch office, branch office, and business office in the prefecture outside the city.

• Application forms, certificate of family register or abstracts, photographs, identification documents, last passport, etc.

If you have a permanent domicile in the city, you can get a certificate of family register or an abstract at the ward office.
