Initiatives related to medical and nursing care cooperation

In cooperation with the Yokohama Medical Association, Yokohama City has established and operated a “Home Healthcare Cooperation Base” in all wards to provide support to doctors who are responsible for home medical care and medical support to care managers who are responsible for home care.
Since January 2015, Kanazawa Ward has established the Kanazawa Ward Home Health Care Consultation Office (external site) within the Kanazawa Ward Sanshi Kaikan.
In addition, we promote cooperation between home medical care and nursing care in the ward through the implementation of multi-occupational cooperation meetings and human resource development training for home medical care. Cooperation between multiple occupations is indispensable for inhabitants of a ward to receive stable medical and nursing care services at home.

In March 2017, we issued the "Cooperation Handbook Kanazawa-Medical Care and Nursing Care Supported by Everyone" as a tool to promote multi-occupational cooperation. .
Please use it as a handbook for those who are in charge of medical and nursing care cooperation!

★At the Kanazawa Ward Home Healthcare, Nursing Care Organizations and Institutional Liaison Meeting held in August 2015, the proposal was made to "create concrete tools to further promote cooperation."
★There are no specific tools in the handbook.
Kanazawa Ward is promoting the use of information sharing sheets created by City of Yokohama, Medical Care Bureau at the time of hospitalization and discharge. We are also promoting the use of Kanazawa Ward's own sheet for palliative care.
Revision of the handbook with the voice of everyone will help promote cooperation.
Even now, Kanazawa Ward's efforts for medical and nursing care cooperation are progressing every day.
