Subsidies for transportation expenses (physical, intellectual, and mental) for facilities, etc.

Subsidize transportation expenses (*) for pick-up and drop-off assistants such as persons and their families required to attend Psychiatry Day Care through facilities and other facilities (excluding elementary school student facilities) registered under this program.
※Excludes the amount borne by other systems.

1 Target person

Persons with disabilities who are 15 years of age or older who live in Yokohama City, who go to the office or go to the hospital to receive the following business, and are mainly public transportation (trains / buses) or private cars (four wheels, * Only when there is no means of commuting other than private cars due to the situation of disability)

2 Billing and Payment Period

Through facilities, etc., billing and payment will be made twice a year for the first half (April 1-September 30) and the second half (October 1-March 31).

(1) About subsidy unit price per one way to tsusho

The subsidy unit price per tsusho means that the main place of residence (the place where the majority of the number of days of living is occupied and the base of life) and the tsusho facility (if you receive a transfer by facility vehicle from the middle) This is the amount per one-way determined in advance based on the main tsusho method (tsusho method and tsusho route) up to the pick-up point).
Even if multiple tsusho methods are assumed, the unit price is determined based on one of the main tsusho methods. When claiming transportation expenses, even if there is a day when you actually go to another route, you will be paid based on the determined subsidy unit price.
(If it becomes normal to use another tsusho method, a change application is required.)

In the case of public transportation (trains and buses), the subsidy unit price is 1.5 times the normal fare for the tsusho person, reflecting the discount for the disabled, etc. The number of rides that have been added is eligible for the grant). In addition, the amount equivalent to the 6-month commuter pass fee is determined as the upper limit.
In the case of a private car (four-wheeled), the distance from the main residence to the shortest route such as tsusho facilities is 20 yen (rounded up to less than 1 km).

However, the subsidy unit price is economic efficiency (amount) and rationality (speed, convenience, etc.). From the viewpoint of disability, the city will confirm a reasonable tsusho route or the city. In addition, in the case of a route that is very different from the normally considered route due to disability (such as making a detour), a reason is required.

(2) Expenses not covered by the grant

The following are not eligible for the grant.

  1. Fare for transportation (buses operating in the city, municipal subways, Kanazawa Seaside Line) that can use tickets, etc. for those who meet the requirements for obtaining a special welfare ticket or a special elderly ticket. (Not applicable regardless of whether or not you have actually obtained a ticket, etc.)
    Requirements for obtaining a special welfare ticket: Handbook for the physically handicapped 1st to 4th grade, handbook for love A1 to B2 (including judgment), handbook for mentally handicapped 1st to 3rd grade
    Requirements for obtaining a special ride certificate for the elderly: 70 years old or older
  2. Transportation expenses for pick-up assistants when a tsusho attends with the assistance of a guide helper of the "Disability Movement Support Project"
  3. Transportation expenses during the trial period
  4. Transportation expenses when going to a facility that provides employment transition support in order to receive an assessment (employment assessment) for the use of the employment continuation support type B payment decision service
  5. Transportation expenses for social security recipients visiting Psychiatry Day Care, Psychiatry Short Care, Psychiatry Night Care, and Psychiatry Day Night Care.

Unit price determination applications and billing procedures are performed by tsusho facilities, etc., which have been delegated office work from the tsusho.
In that case, we will use the "tsusho transportation expenses subsidy system".
※We do not accept applications or requests from individual residents.

1 Facility Registration
(1) Submission of facility registration sheet

This is a pre-registration so that only facilities that perform billing work can use the tsusho transportation system.
This registration is required only once for each facility.
In order to ensure security, the number of PCs that can use the system is limited to two per facility.
At the time of facility registration, it is necessary to register the MAC address of the PC that uses the system.
Please fill out the following file and send it by e-mail.

In addition, when there is a change in the registered contents, it is necessary to change the facility information as appropriate. Change work can be done on the system for "TEL number", "FAX number", "e-mail address", "name of facility manager", and "account information". <System Usage Manual>

Other items cannot be done on the system, so please fill out the facility information change sheet and send it by e-mail.

(2) Obtain a login ID and password (mailed from the city)

Based on the contents of the facility registration application form, after registering the facility in Yokohama City, a login ID and password will be mailed from Yokohama City when using the system for each facility.

For users who have started a tsusho, users will understand the tsusho route information and have them submit a power of attorney (seal).
※If your main residence is different from Address in resident certificate, you will need to submit a certificate of residence (such as a utility bill receipt) to the city when applying for unit price determination.

(2) Application for unit price determination (using the tsusho transportation cost subsidy system) <System usage manual>

Based on the tsusho route information, apply for the subsidy unit price by the system by the 20th of the month following the start of the tsusho.

  1. Based on the tsusho route information, apply for the subsidy unit price by the system by the 20th of the month following the month in which the tsusho starts (you do not need to mail an application to Yokohama City).
    ※For those who started tsusho in March and September, the application deadline is April 10 and October 10, respectively. (Because it is necessary to enter billing by April 20 and October 20) ※Even if the unit price application and billing are not in time, the unit price application and request can be made retroactively for a certain period of time.
  2. Print out the application information from the tsusho transportation expenses system and check with the user whether there is any error in the application details (personal information and tsusho route). If there is an error in the application content, please correct the application content on the system and apply again.
    ※The subsidized unit price is displayed on the printed application information, but be careful because if there is an error in the application details, the unit price after examination and decision in Yokohama City may change from the time of application.

(3) Subsidy unit price determination (displayed on the tsusho transportation expenses subsidy system) <System usage manual>

The unit price application information received by the 20th of each month will be reviewed by Yokohama City at the end of the month, and the determined unit price will be displayed on the system at the beginning of the following month.
If there is an error in the application details, the unit price at the time of application and the determined unit price may differ in the amount, so please check with the system to see if there is a difference between the application unit price and the determined unit price.
※If the user cannot be identified in the information submitted (name, date of birth, Address, etc. have a major error), the application information may be returned.

(4) Print out the determined unit price and confirm it with the user

For users whose application unit price and determined unit price are different, print out the system screen and tell the determined unit price to the user.
※Users whose application unit price is equal to the determined unit price have been confirmed with the user at the time of application, so there is no need to reconfirm after the decision.

(5) Change of grant unit price <System Usage Manual>

If the disability situation or the main tsusho situation changes, you will need to apply for a “unit price determination” (change application) again.

(Example)

  1. When the resident's notebook status changes (when a new notebook is acquired, when the grade of the notebook changes, when the notebook is abolished, etc.)
  2. When the main place of residence of the resident changes (for example, when moving)
  3. When the main tsusho route changes (for example, when the transportation used mainly changes)
  4. When the main attendance status of the pick-up assistant changes (for example, when the pick-up assistant needs to be accompanied (unnecessary), etc.)

(1) Input billing information and printing invoices (system) <System Usage Manual>

Enter the billing information (number of tsusho for each user, etc.) in the tsusho transportation expenses system and print the invoice. For each user and the total billed amount, the tsusho transportation expenses system automatically calculates.
Input for the first half (from April 1 to September 30), from October 1 to October 20, and the second half (from October 1 to March 31) from April 1 to April 20. Please go from 1 to April 20.

(2) Submission of invoices and power of attorney (mail)

Seal the representative's seal on the printed invoice and mail it to Yokohama City with a power of attorney (copy). (Submission of a power of attorney (copy) is made once a year (at the time of the first request made to the office))
Mailing is valid for postmarks, for the first half (from April 1 to September 30), from October 1 to October 20, and for the second half (from October 1 to March 31) Please go from April 1 to April 20. (* If the submission deadline is a closed agency day, the next open agency day will be the submission deadline (postmark valid))

※We will examine the invoice in Yokohama City and contact the facility if there is any omission in seal. In that case, the facility should resubmit the revised version of the invoice to Yokohama City as necessary.

(3) Payment of transportation expenses

  1. Transfer the amount stated in the invoice from Yokohama City to the account of each facility. In addition, it is possible to check the scheduled transfer date to each account using the tsusho transportation expenses system. (We will be able to confirm it in mid-November for the first half and mid-May for the second half.)
  2. Please hand over the transportation expenses transferred from Yokohama City to each user and receive a seal in the payment ledger. (preserved for 5 years)

Up to the final submission deadline in the table below, you can make a request by submitting a “Reason for Delay” that describes the reason for the delay in submission in addition to the normal billing documents.
