About application of designated medical institution (new, change, update, suspension, abolition, resumption, decline)
When a medical institution in Yokohama handles independence support medical care (psychiatric outpatient medical care), it is necessary to receive the designation of Yokohama City and become a designated medical institution.
For medical institutions other than Yokohama City, please contact the local city (designated city) or prefecture.
New application

◎The application deadline is 20th of every month, and in principle it will be a designated medical institution from the 1st of the following month.
The medical treatment before becoming a designated medical institution is not eligible for the system. (No retroactive application is possible.)
◎It is not a requirement for hospitals and clinics to have Medical consultation fee, such as Psychiatry or psychosomatic medicine, but a requirement for doctors who are primarily responsible for them to have at least three years of experience in the mental field (including epilepsy).
◎For pharmacies, there is a requirement that "the manager (managed pharmacist) has experience as a manager (managed pharmacist) at another designated medical institution in the past."
If a pharmacist reporting as a manager (managed pharmacist) at a new pharmacy has no experience as a manager (managed pharmacist) at another designated medical institution in the past, the designated month will be delayed.

Application for renewal
Designated medical institution needs procedure of designated update by six years after designation.
Notifications are sent to medical institutions whose renewal deadline is approaching.
If there are any changes at the timing of renewal, please refer to the “Application for Change” above and submit required documents together.

If many people come to the ward office window to apply in a short period of time, we may not be able to respond. We apologize for any inconvenience, but we appreciate your cooperation.
★Please check the patient's recipient ID and the upper limit management slip at each consultation. If you forget to show your beneficiary ID, the system will not be applied. (Refunds will not be available at a later date.)

Handling of recipient IDs and upper limit management slips
Recipient ID
◎The recipient ID of Yokohama City is B7 size and the color is yellow.
◎The public expenditure payer number for independence support medical care (psychiatric outpatient medical care) in Yokohama City is "21146022".
◎Hospitals, clinics, pharmacies, and home-visit nursing stations that are not listed on the recipient ID are not eligible for the system.
◎Even if the pharmacy is listed on the beneficiary card, if the medicine is prescribed by a prescription from a hospital or clinics that is not listed on the beneficiary card, independence support medical care will not be applied to payment at the pharmacy. In addition, if the prescription date of the medicine is out of the validity period, independence support medical care will not be applied even if the dispensing date is within the validity period.
◎If you forget your recipient ID, you will not be eligible for the system. (There is no refund in Yokohama City.)
◎Changes to medical institutions can be made at the ward office window (same day) or by mail. (Application from the patient is required.)
Changes will be made from the date of receipt of the application and cannot be retroactively applied.
◎In principle, one hospital or clinics can be designated, two pharmacies, and one home-visit nursing station can be designated.

Out-of-pocket upper limit management slip
◎Yokohama City's self-pay upper limit management slip is a B7 size, light blue booklet. (It is issued separately from the yellow recipient ID.)
◎It is not issued to those who do not have self-pay (social security recipients) or those who do not have an upper limit.
◎For those who have the upper limit in the "monthly burden upper limit" column of the yellow recipient ID, please fill out the upper limit management slip when collecting the copayment.
◎After the monthly limit is reached, there is no user burden. In that case, it is not necessary to enter the upper limit management slip.
◎For those who have both independence support medical care certificate and Medical Certificate with severe disabilities, please give priority to independence support medical care and apply medical care for severely disabled people to the remaining co-payment even if you apply independence support medical care. At that time, there is no co-payment of the recipient himself, but for the upper limit management of independence support medical care, please fill out the co-payment that occurred without using medical care for severely disabled people on the upper limit management slip . (If you have a Medical Certificate for infant, please do the same.)
◎If you do not bring the upper limit management slip (forget it, etc.), please print out this auxiliary paper (Excel: 11KB) and use it.
