In February 2025, Yokohama City launched an online application for independence support medical care (psychiatric outpatient medical care).
※Online application refers to an application made on a smartphone or PC.

1.Application for renewal (not required for submission of medical certificate)
This is a procedure for those who already have a recipient ID of Yokohama City and want to continue using the system after the expiration date.
Conditions for those who can apply for "renewal" online (those who meet all of the following conditions)

◆After accepting the online application, if there are no deficiencies or shortages, we will mail you a “Independence Support Medical Care (Mental Outpatient Medical Care) Payment Authorization Input Confirmation Form (Personal Copy)” in about two weeks. During the period until the recipient ID arrives, please show the copy to the hospital, pharmacy, etc.
◆If your application is accepted, we will send you a recipient ID. (It takes about 1 to 2 months from the application.)
◆If you are unable to apply for the system even if you present your copy to a hospital or pharmacy, please state that you would like to receive a refund to the hospital or pharmacy as soon as you receive your beneficiary ID.

4.Out-of-city transfer application (application to change from identification of recipient except Yokohama to identification of recipient of Yokohama-shi)
This is the procedure for those who have a recipient ID issued by another local government and who have moved to Yokohama within the validity period.

・This is the PIN set by yourself when you receive your My Number Card at the municipal office.
