[Medical institutions in the city] Application for creating an infectious disease surveillance system (for fixed-point reporting only) account

　If the reporting method is faxed at a facility designated as a fixed-point medical institution, you can report online using the infectious disease surveillance system (hereinafter referred to as the system).
　If you wish to use the system, please fill out the application form or e-mail body with required items and contact the Health Research Institute.
