[For medical and nursing care personnel] About the dissemination and enlightenment of ACP

In Yokohama City, we have created tools that can be used by medical and nursing care professionals to spread and enlighten ACP.
Please use it when enlightening citizens about ACP.

It was created under the supervision of medical and nursing professionals as a tool that can be used by medical and nursing staff and administrative staff to educate ACP for citizens. Please use it when enlightening ACP and "What if Notebooks" at lectures for citizens or dispatch lecture/off site lecture.

The purpose is to encourage citizens to understand the significance of ACP and to think and discuss in advance.

The notebook has a reading manuscript.

There are two types, lecture and work. Both have read manuscripts and can be used easily.
Glossarys for explainers and URLs for reference materials are also posted.

（１） Lecture "If you have a notebook, life meeting!"
　Contents: Understanding ACP, how to communicate your feelings using the "Homebook", etc.
　Time required: About 20 minutes

（２） Work Edition "Drawal of Value"
　Contents: Work to think about what you value and experience the fluctuations of your feelings.

　Emphasize that "in advance" is conveyed so that the importance of discussing in advance is conveyed.

The feeling of "I don't want to think", "I don't know", "I can't decide" is also important. Don't overdo it and tell them that you're good at your own pace.

〇 Arrangement of the contents
　It is forbidden to modify or add slides without permission. However, it is possible to omit a part of the slide or arrange the reading manuscript according to the time.
