１．Request a “practical certificate” to the work facility or corporation
※The "Practical Certificate" is also used to apply for a kindergarten license using the special kindergarten system. Please use the form specified by each prefecture.

２．List required items in the “Application for Issuance of Facility Certificates for Special Nursing Care System (PDF: 123KB)” and “Certificate for Facilities for Special Nursing Care System (PDF: 100KB)”
※If the “Certificate of Facilities subject to the special childcare system” is specified in each prefecture, please use the applicable format.

３．Prepare a reply envelope (preferably long-form No. 3 envelope)
