It is information when applying for "Mental Handicapped Health and Welfare Handbook" by "mail".
Please confirm what you need for the application and send it to the paperwork center by mail. (You can't bring it directly.)

◆In the case of mail application, the date on which the paperwork center receives the documents = the application date. In the case of a new application, the notebook will be valid from that date.
◆The paperwork center is closed on Saturdays, Sundays, holidays, and the year-end and New Year holidays, so the documents received on that day will be the day on which the documents were received on the next open agency day.
◆If the contents of the application are incomplete or there are insufficient documents to be submitted, the paperwork center will contact you. Type of application

1.New application

This is required documents, the first person who applies for the Mental Handicapped Health and Welfare Handbook.
Person applying for independence support medical care (psychiatric outpatient medical care) system at the same time, please move to (simultaneous application at ward office) (simultaneous application by mail).

Explanation of simultaneous application

Judgment of the Mental Handicapped Health and Welfare Handbook is based on a medical certificate, but if you have received a disability pension or special disability benefit for reasons of mental illness, you can submit a copy of your pension certificate (without submitting a medical certificate). In that case, the grade of disability pension is the grade of the notebook.

◆Please choose either A3, 1 or A4 or 2 depending on the printer situation. (Please enlarge the A3 version to A3 and print it out. When printing in A4.2 sheets, print with two sheets on one side instead of double-sided printing. )
◆Please make sure that six months have passed since your first visit (Shoshin) date.
◆I will not apply for the independence support medical system this time, but it may be done at a later date.…In that case, please also write items 9-11 of the medical certificate.

◆From January 2024, we will refer to the pension Office with My Number.

2.Application for renewal and reapproval

If the person who already has a notebook renews within the validity period (= renewal application) or if the person who has a notebook but has expired applies again (= reapproval application) required documents.
If you apply for independence support medical care (psychiatric outpatient medical care) at the same time, please move to (simultaneous application at the ward office) (simultaneous application by mail).

◆You can apply for renewal at least three months before the expiration date.

In the case above, the expiration date is March 31, 2022. In that case, you can apply for renewal from January 1, 2022, three months ago.

※For February, the year up to the 29th is replaced with February 29th.
※Applications cannot be accepted because the paperwork center is closed on Saturdays, Sundays, holidays, and New Year's holidays. Please apply as soon as possible.
◆We have not sent a notice of renewal time from Yokohama City. Please check the expiration date written in your notebook and apply for renewal so that you do not forget it.
◆If you apply for renewal by the expiration date, the period will not be interrupted, but it will take about two months from the application to the issuance of your notebook. (It depends on the time and content of the application.) If you present your notebook and use other services, you may not be able to receive the service because you do not have the notebook, so it will be 3 months before We recommend applying immediately.

3.Application for class change

This is required documents if you want to change the grade of your notebook.

◆The grades of the Mental Handicapped Health and Welfare Handbook are three levels: grade 1, grade 2, and grade 3. (The first grade is the heaviest and the third grade is the lightest.)
◆If you wish to change your grade, you will be required to submit a copy of your medical certificate or pension certificate and undergo a review again.
◆If the grade change is approved, the validity period will change from your current notebook.
◆If the grade change is not approved (= grade did not change), we will send you a disapproval notice.

4.Out-of-city transfer application

This is required documents when I moved from a municipal office other than Yokohama to Yokohama.

If the previous municipality has been issued a Mental Handicapped Health and Welfare Handbook, the contents will be taken over until the expiration date of the handbook.
If less than three months before the expiration date and you have not renewed at the previous municipality, you need to apply for renewal in Yokohama City.

5.Application for change of items described

This is required documents when the name and address (moving in Yokohama) change.

7.Card style change application

This is required documents if you currently have a paper notebook and want to change it to a card style.
※If you want to change the card style to paper style, it is a re-grant application.

8.Notification of return

This is required documents when you don't need a notebook and return it.

9.About the flow from application to issuance of notebook

◆It takes about 2 months from the application to the issuance of the notebook. It depends on the timing and content of the application.
◆The application will be accepted by the paperwork center, and if there are no deficiencies or shortages, we will send you a form called the reception confirmation slip (A4). This form does not mean that "the delivery of the notebook has been decided" because the notification that the application has been accepted.
◆In the case of "change of information" such as change of name or address, we will send you a notice from the paperwork center when you are ready. Please bring what is written there and get it corrected at the ward office Elderly and Disabled Support Division window.
◆As a result of examination, when grant of notebook is decided, we send guidance called "news of grant of mental patient health welfare notebook" from ward office. Please bring what is written there and receive a notebook at the ward office Elderly and Disabled Support Division window. (Only card style change, we mail notebook directly from health consultation center of city heart.)

I have been short since I was diagnosed with mental illness. Can I apply for a notebook?
A

If more than six months have passed since the date of first visit (Shoshin), your notebook will not be approved. Please wait for 6 months to apply.
※If you are transferred, "first visit (Shoshin)" is not the day of first visit (Shoshin) at the hospital you are currently attending, but the day you were first diagnosed with the disability.

I am currently hospitalized, can I apply for a notebook?
A

You can apply even if you are hospitalized. However, the independence support medical care (psychiatric outpatient medical care) system that can be applied at the same time with the same medical certificate is a system for outpatient visits, so if you are still hospitalized at the time of examination and there is no prospect of discharge, independence support medical care will not be certified It becomes.

Is intellectual disability covered by the Mental Handicapped Health and Welfare Handbook?

Intellectual disability is not covered by the Mental Handicapped Health and Welfare Handbook because there is a separate system called the "Love Handbook".

medical expenses, such as Psychiatry, is cheaper by the independence support system (psychiatric outpatient care) system.

Photographs must meet the following conditions.

◎The upper body is shown so you can identify yourself.
