Identity verification (My Number Card, driver's license, employee health insurance qualification confirmation or health insurance card, etc.)
Form for change application of Seal Registration (replacement of certificate, loss of certificate, abolition, loss of private seal) (Please print the application form from the Yokohama City Electronic Application and Notification Service page (outside site) or obtain it at the ward office window.)

When proxy/agent proceeds

proxy/agent's identity verification (My Number Card, Driver's License, employee health insurance Certificate of Eligibility or Health Insurance Card, etc.)
Power of attorney created by the person himself / herself (Please make it within 2 weeks referring to a sample of power of attorney (PDF: 385KB))
Form for change application of Seal Registration (replacement of certificate, loss of certificate, abolition, loss of private seal) (Please print the application form from the Yokohama City Electronic Application and Notification Service page (outside site) or obtain it at the ward office window.)
