For those who have a My Number Card

If you are a Japanese citizen, you will need to apply for the continued use of your My Number Card at the ward office Family Registry Division window in your ward by the day before departure.

Notes

Procedures for National Health Insurance, National Pension Plan (Kokumin Nenkin), The Long-term Care Insurance, Child Allowance, etc. may be required at the counter. Please check in advance.
When you report at window, please see this "at window (moving outside the country)".

１．Notification of transfer

２．A copy of the identity verification document of the person submitting the notification

My number card
Driver's license
Passport
Health insurance card

Notes

For a detailed list of identity verification documents, please refer to the identity verification at the time of notification of change of residence in family register and when requesting various certifications.
The identification documents may contain the Address before the change of Address.
When sending a copy of your health insurance card, please send the insurer number and the insured person symbol / number so that they cannot be seen by black paint etc.

３．(In case of notification by voluntary proxy/agent) Power of Attorney

Please refer to the sample and create it yourself.

For inquiries regarding the National Health Insurance, The Long-term Care Insurance, National Pension Plan (Kokumin Nenkin), Child Allowance, etc. after notification of transfer abroad, please contact the ward office section in your ward directly.
Seal Registration will automatically expire on the scheduled date of transfer, so there is no need for additional procedures.
