Application Forms and Guides (Related to bath tax)

1. We do not provide all application and notification forms.
This page provides some application forms. For forms that are not provided, please use those distributed at the application destination (in charge of bath tax, Finance Bureau Corporate Taxation Division).

2. In principle, please submit the declaration form by mail or by bringing it to the submission destination (Finance Bureau Corporate Taxation Division, bath tax).
※You can also submit the "Spring Bath Management Declaration Form" by e-mail.

3 "Adobe Reader" is required. I need it.
Application and notification forms are provided in PDF (Portable Document Format). To view and print PDF files, Adobe Reader, which is distributed free of charge, is required. If you do not have it, download and install Adobe Reader (external site) from the Adobe Systems Incorporated site before using it.

4 Use plain paper for printing.
Please use A4 size white plain paper for the declaration form. Please note that if the size and printing surface are different, it may not be accepted. Please do not use thermal paper, backing paper, colored paper, etc.

5 Please note the following points when printing the application / notification form.
When printing the declaration form, display the form on the screen and press the print button of Acrobat Reader. You cannot print the application / notification form with the print button of the browser.

6 Please check the latest information.
The report form may be changed (revised) due to revisions to the ordinances and rules, so please check the latest information when deserving the report form.
