※In the case of a change pertaining to the receiving tank, a notification of the change of the applicable receiving tank is also required.

About building corresponding to specific building, it is necessary to submit "annual management execution report" of last year and "annual management plan" of this year every year.

When there is facility which did not meet building environmental hygiene management standard, please submit "specific building maintenance situation confirmation vote" (Excel: 16KB).
