Cooperation of stakeholders is indispensable so that inhabitants of a ward can live home medical treatment with peace of mind. In order to create an environment in which multi-professional (medical care, nursing care, welfare, etc.) supporters who support home medical treatment can cooperate better, we have held regular meetings since fiscal 2007 (representatives from each group) and exchange meetings (commonly known as Seyamaru Cafe Medical, Welfare, and Nursing Care).

・Seya Ward Medical Association
Established on April 1, 1971. Seya Ward Holiday Emergency clinics, home-visit nursing station, home support office (care plan creation) and helper station opened. In charge of cancer screening, infant health check-up, school doctors at elementary, junior high and high schools in the city, nursery school doctors, and counselors at local care plazas.
・Seya Dental Association
Established on April 1, 1970. We are active with a mission of oral care for a lifetime, such as infant dental checkups, periodontal disease prevention classes, and visiting dental care for bedridden elderly people.
・Seya Ward Pharmacists Association
Established on April 1, 1995. We are working to contribute to the promotion of welfare and welfare for local residents by promoting the advancement and development of the pharmacy and the pharmaceutical industry.
・Seya Ward Home Visiting Liaison Committee
Established in 2003. It consists mainly of visiting nursing stations in Seya Ward. Saint Marianna University School of Medicine, Yokohama City Seibu Hospital General Counseling Department Home Care Section, Seya Caremanette and Seya Ward Office are also participating. Conducted liaison meetings to exchange information, improve the quality of home-visit nursing services, conduct training and research for business development, and cooperate with related organizations with the aim of contributing to the sound development of home-visit nursing services.
・Seya Care Manet
Established in 2002. care manager (care manager) who lives and works in Seya Ward. Conducted training and information exchange meetings to improve the quality of care manager.
・Seya Ward Community-based Service Office Liaison Meeting
Established in 2012. Participate in dementia-responsive cohabitation care (group home) and small-scale multifunctional home care. Conducted information sharing meetings and workshops with the aim of improving services.
・Seya Ward Visiting Nursing Care Facility Liaison Meeting
Established in 2013. Conducted training and liaison meetings with the aim of promoting horizontal cooperation between offices, improving the quality of home-visit nursing care services, and strengthening cooperation with other occupations.

Hospitals, home care support clinics, etc. participated.
・Community General Support Center
An organization that supports the elderly to continue living in the area where they live. Public health nurses, social workers, and chief care managers are available. Care consultation, care prevention, adult guardianship system, certification for long-term care, administrative services for the elderly in Yokohama, introduction of local events and salons, etc. There are five places in Seya Ward (Futatsu Bridge, Akuwa, Nakayashiki, Shimoseya, Futatsu Bridge Second). All of them are located in the community care plaza.

At present, welfare services for the elderly are based on the long-term care insurance system. At this liaison meeting, we have been creating a booklet to make people feel familiar with the difficult-to-understand long-term care insurance system, and examining ways to make it easier for multi-disciplinary supporters to cooperate. I would like to introduce you to the following.

When using the The Long-term Care Insurance service, the care manager adjusts the use of the service.
The care manager listens to the wishes of the person and his / her family, and introduces appropriate services in cooperation with the user's physician and related organizations.
