Vaccinations in general

When we receive routine immunization for child in medical institution outside the city
・ If you receive routine vaccinations at a medical institution outside the city, you will need to apply in advance at the ward office Health Promotion Section. Based on the Immunization Act
We issue "vaccination conduct request book" which is document which clarified that conduct responsibility is in the mayor of Yokohama.
・ In principle, the vaccination fee will be borne by you.
・ If you have a resident registration in Totsuka Ward as of the date of vaccination, you can apply online from the link below.
※User registration of the Yokohama City Electronic Application System is required for use.
※After applying, the ward office Health Promotion Section will call you if necessary.
・It takes about one week from application to sending the documents. It may take longer than usual depending on postal conditions, weekends and holidays, etc.

・ If you have difficulty applying online, please contact us.
※Before receiving vaccinations, be sure to read the Yokohama City website or the Vaccinations Bookmark for Children, which reaches a 0-year-old child at home.
After having you understand contents well, I would like inoculation.
When applying, please check with Vaccinations in charge of the municipality where the medical institution you wish to inoculate is located about the following items in advance.
・Do you provide "Vaccinations expense subsidies" for those who are staying on their way home? (If you have a system, please use it.)
・Address of the vaccination request form (“Medical institution director”, “Municipal mayor (local government)”, etc.)
・How to submit a vaccination request form
("Send to your accommodation and bring it to a medical institution at the time of vaccination", "Send directly to the local government department", "Send directly to medical institutions", etc.)
・Whether you need a pre-examination card (vaccination coupons) from Yokohama.
※If necessary, please use the pre-examination card mailed to your home.
If you receive routine vaccinations outside the city due to the following unavoidable circumstances, you can receive a refund (redemption payment) of the cost pertaining to the vaccination by receiving a “Vaccination Implementation Request Form” in advance.
・Homecoming births outside the city (including births of younger brothers and sisters)
・In hospital treatment at a hospital outside the city
