You can apply for the following procedures by mail.

high medical costs applies for payment of large sum of nursing care expenses
Applicants will receive an application form from the Kanagawa Prefecture Senior Citizens' Medical and Wide Area Alliance.
Please write required items on the application form and mail it to the Totsuka Ward Office Insurance and Pension Divisionseal.
Application for payment of medical expenses, dietary expenses at the time of hospitalization, and funeral expenses

Application for grant of limit application certificate, limit application / standard burden reduction certificate
If you wish to apply by mail, please contact the Totsuka Ward Office Insurance and Pension Division Benefits Section by phone from you or your family. After confirming the necessity of the procedure, we will mail the application form as necessary.
※Procedures are not required for households with a health insurance card burden ratio of 30% and taxable income of 6.9 million yen or more, those with a burden ratio of 10% and households with residence tax taxable income.
