For those who do not have a pre-examination card due to transfer or loss
For those who do not have a pre-examination card due to transference or loss, after confirming the past vaccination history at the window of the ward, we will issue the necessary pre-examination card.
①Documents confirming the name, date of birth, and Address of the person to be vaccinated
②Original documents showing past vaccination history (Mother and Child Handbook, etc.)
Please bring your own to the ward office Health and Welfare Division Health Promotion Section (23rd floor on the second floor).

If you cannot come to the counter due to circumstances, we will accept a mail application. Specifically, please see "to person applying for grant of pre-examination vote by mail" (PDF: 172KB).

When receiving routine immunizations for children at a medical institution outside the city
・ If you receive routine vaccinations at a medical institution outside the city, you will need to apply in advance at the ward office Health Promotion Section. Based on the Vaccination Law, we will issue a “Vaccination Implementation Request Form” which is a document that clarifies that the responsibility for the implementation lies with the mayor of Yokohama.
・ As of the date of vaccination, there is a resident registration in Tsuzuki Ward , and regular vaccinations at medical institutions outside the city due to reasons such as returning home outside the city (including births of younger brothers and sisters) or hospitalization treatment at medical institutions outside the city If you wish to receive a vaccination, you can apply for an electronic application from the link below.

※User registration of the Yokohama City Electronic Application System is required for use. After applying, the ward office Health Promotion Section will call you. (It takes about 1 to 2 weeks from receipt of application to shipment.)
・ Vaccination costs outside the city are entirely borne by you, but in the case of the above reasons, you can receive a refund (redemption payment) of the vaccination costs by receiving a “Vaccination Implementation Request Form” in advance. Information on how to refund will be mailed together with the “Vaccination Implementation Request Form”.
