A person who intends to submit a proposal shall satisfy all of the following conditions (1) to (4).

(1)　Being not a person listed in Article 3, Paragraph 1 of the Yokohama City Contract Regulations (March 1964 Yokohama City Regulations No. 59) and a person who has the qualifications specified in the provisions of Paragraph 2 of the same Article.

(2)　Person who is authorized to register in event "computer duties (316)" and "various investigation plans (320)" in Yokohama-shi general competitive bidding qualified person list (related to article, trust) in Reiwa 5.6 thing. Provided, however, that this shall not apply if you are applying for the above event at the time of submitting the application form and the registration is completed before the date of identifying the trustee.

(3)　 Being a person who has not received nomination suspension according to the provisions of the measures outline such as Yokohama City nomination suspension for the period from February 7, 2024 to a specific day of the trustee.

(4)　Within the past five years, you must have a track record of building and operating websites, and have a track record of building navigation functions such as searching for cancer screening medical institutions.

We will conduct a hearing on the proposal as follows.

(4)　Response date and method
　 　We will respond by e-mail by Thursday, February 22, 2024.

※If you don't have a question, I will not send it.

(1)　By March 5, 2024 (Friday), a letter of confirmation of the proposal qualification will be issued by electronic mail to all those who have submitted the application form for participation.
(2)　In addition, a proposal-related document submission request form will be issued to those who will participate. Those who have been approved for proposal qualifications should submit proposals, etc. based on the proposal preparation guidelines.
(3)　By 5:00 pm, 5 days after the day following the dispatch date of the “Proposal Qualification Confirmation Result Notice”, submit a document requesting explanation (free style) to the "Participation Intention Application Form" Please submit to the destination. Motoichi will respond in writing within 5 days (excluding the city hall closing date) from the day following the date of receipt of the above document.

　This contract is a condition that each accounting budget for Yokohama City in 2024 will be suspended by the Yokohama City Council by March 31, 2024.
