I have lost my health insurance card for the medical system for elder senior citizens, so I would like to apply for a reissue.

Issuance of identification card for the elderly medical care insured person (insurance card) was finished on December 1, 2024.
If you lose your insurance card after December 2, 2024, you will be issued a “Qualification Confirmation Form”.
Please apply for the grant to the ward office Insurance and Pension Division National Health Insurance Section.
What is required for application
・Application for the issuance of a medical certificate for the latter-stage elderly medical qualification and a description of optional items
・Identity verification documents (If someone other than the person or a relative of same household applies in proxy/agent, the identity verification documents of the authorized person are required.)
Documents with a face photo issued by a public office such as a driver's license, my number card, passport, etc.
・Power of Attorney (required if someone other than the person or a relative of same household applies in proxy/agent)
・If an adult guardian has been appointed, a copy of the certificate of entry, etc.

・As a general rule, it will be delivered by mail and it will take a few days to arrive. If you plan to receive a medical examination immediately, you can issue a “medical certificate” as a substitute for your qualification confirmation, so please apply to the ward office Insurance and Pension Division National Health Insurance Section.
・If you lose your insurance card or qualification confirmation due to theft, etc., please send it to the police.
