Where to apply for independence support medical care (psychiatric outpatient medical care)?
You can apply by mail at the following office.
In addition to mailing applications, we will also accept applications at the Elderly and Disabled Support Division of each ward office.
Please refer to "How to apply" on the related website.
If you do not have an Internet environment, please contact the Mental Outpatient Medical Care and Notebook Processing Center.
