Yokohama City has created a ward version of ending notes for each of the 18 wards.
An ending note is a note that looks back on your life so far and describes your thoughts on how you want to walk in your future life. We provide necessary information such as health promotion, nursing care, and adult guardianship systems.
You can participate in the ward version of the ending notes in each ward, or receive explanations at the counters of Elderly and Disabled Support Division, Community Care Plaza, etc. in your ward.
