Facilities for the elderly and disabled
COVID-19 infection
If a COVID-19 infection (including suspicion) occurs in facilities for the elderly or disabled and the following 1 report is required, please fill out the following required items form and submit it by e-mail or fax to the address 3 below.
※When submitting by e-mail, please write the subject "[Name of the facility] COVID-19 infection status Report".
1 When a report is required
(1) In the event that two or more seriously ill patients are suspected according to COVID-19 infection occur within a week
(2) When more than 10 patients (including suspicions) in COVID-19 infection occur or more than half of all users
(3) Even if it does not fall under (1) or (2) above, it is suspected that infectious diseases, etc. that exceed normal outbreak trends, and in particular the facility manager deems it necessary to report.

If Health and Welfare Center deems it necessary, we may visit the facility (on-site survey) and listen to it. In addition, we may ask you to prepare the following items in advance during the on-site survey.
In addition, in the case of infectious gastroenteritis, please cooperate with stool test to determine the causative pathogen.

If an infectious disease such as COVID-19 infection, influenza, gastroenteritis, etc. (including suspicion) occurs at a childcare facility and the following 1 report is required, please fill out the following required items form and submit it to the following 3 by e-mail or fax, and let us know by telephone.

(4) Regarding “Infectious diseases (Class 1 to 3) and measles / rubella specified by the Infectious Diseases Law”, when even one person has occurred or is suspected.
