Passport (passport) application form

The Kanagawa Ward Office provides information on passport applications and distributes applications (for 5 years and 10 years).
Distribution locations are general information (1st floor of the annex) and Public Relations Section Ward Administration Promotion Division (503 window on the 5th floor of the main building).

※We do not accept applications.
