What is independence support medical care (fostering medical care)?
Part of the medical expenses when a child under the age of 18 is treated at a designated medical institution to reduce or restore physical disability.
This is a system that pays public funds. (It will be provided in kind. Only the treatment equipment cost will be refunded at a later date. )
Out-of-pocket expenses are 10% in principle. However, there is a monthly upper limit on the out-of-pocket amount depending on your income.
In addition, households (same insurance) with a certain income or more (municipal tax income percent before special tax credits such as housing loans and donation deductions is 235,000 yen or more)
If you belong to "severe and continue" and do not fall under "severe and continuous", you will not be covered by public expenses.
In principle, prior application is required.
However, if there is an unavoidable reason, such as when the procedure is not completed in time due to emergency surgery, we will accept applications after the fact, so please consult us.
In addition, if we are unable to obtain a written opinion on nursing care due to the influence of COVID-19, we are accepting applications after the fact.

Those who are eligible
Parents are children under the age of 18 who have Address in Yokohama (until two days before their 18th birthday) who have a disability.
Or there is a disease that is considered to leave a disability in the future if left unchecked, and treatment such as surgery at a designated medical institution for nursing care.

Details of Benefits
The co-payment of insurance medical treatment (for which employee health insurance applies) for the treatment of the target disability at designated medical institutions is eligible for payment.
※Expenses not covered by employee health insurance, such as differential bed charges, and meals at the time of hospitalization are not included. (For households receiving social security, meals at the time of hospitalization are also available.

Except for treatment equipment costs, medical expenses, which has been paid (payment) at medical institutions, is not eligible. You can't get a refund later.
The cost of creating a treatment corset and other equipment necessary for nursing care and paying the full amount (100%) is first to employee health insurance.
When you request and receive a payment decision letter from employee health insurance, please apply for a refund at upbringing medical care.
Period covered
The target period varies depending on the type of disability and the type of treatment.
Surgery and postoperative outpatient treatment are generally within three months, and are the expected period of medical treatment described in the nursing care statement by the designated physician of the nursing care medical care.
For heart dysfunction, only the length of hospital stay with surgery (within 3 months), but anti-immunotherapy after heart transplant surgery is also applicable to outpatients.
Hemodialysis therapy, anti-immunotherapy after kidney, liver, and heart transplantation, central venous nutrition for small bowel dysfunction, orthodontics and language training associated with palate.
Some are eligible for up to one year, such as physical disability treatment, rehabilitation, and anti-HIV therapy. Continuation with these treatments
If necessary, you can apply for recertification.

1.Documents required for all people
・Copy of health insurance card…Those of the child and the insured (in the case of National Health Insurance, the whole family member)
・My Number Confirmation Documents…All family members who join the same employee health insurance as their child
・Identification documents for those who come to the application window…A photo with a face photo issued by a public organization (driver's license, passport, my number card, etc.)
If you don't have a photo, you need two types.
2.Documents required only for those who apply

・The taxable year for which the insured person in employee health insurance to which the child is enrolled applies (depending on the scheduled start date of treatment for nursing care) applies.

Flow after application
1. If the application is accepted and there are no defects in the documents, the Yokohama City Hall will conduct a review, and if approved, it will usually take about one month from the ward office.
“Nurturing medical care certificate” and “Self-pay upper limit management slip” (social security has no self-pay, so the self-pay upper limit management slip is required.

2. When you receive the training medical care certificate, you will receive a health insurance card and a medical care certificate, at the time of payment at a medical institution.
Please present the self-pay upper limit management slip together. (If you have a Medical Certificate for infant, etc., a medical institution in Kanagawa Prefecture.
If so, you can use it together.
Without the presentation of medical care certificate, upbringing medical care will not be applied, and it will be treated as general insurance medical treatment (20 to 30% of self-pay).
There is no charge for 10%.
3. If you are charged for treatment equipment, please submit the claim documents to the application window after receiving the ID of the recipient.

4. If upbringing medical care is not certified, the ward office will mail a “Independence Support Medical Non-certification Notice” to your home.
(Delivery by ordinary mail.)
5. One and a half months after the application, without any notice from the ward office regarding inadequate documents, etc., training medical care certificate or independence support medical care
If you do not receive any of the non-certified notices, please contact the ward office Children and Families Support Division where you applied.
