Submission of missing documents pertaining to application for disability welfare services, withdrawal of application
If there are missing documents at the time of applying for disability welfare services, you can bring them to the counter and submit them using the following methods. You can also use the following form if you want to withdraw your application.

1.Online submission
Please fill in the required items form below and submit it with the image data of the missing documents attached.

※You do not need a My Number card to submit missing documents.
※It is also possible to apply for proxy by someone other than the user himself.
2.Submission by mail

Please send the missing documents + memo stating the following to the ward office of the application by mail.
※If you receive information about missing documents at the ward office window, you can enclose the information instead of a memo.

・Date of birth
・Applicant Address
・Recipient number ※No new application is required.
・Name of the person who submitted the missing documents
[Notes]
・Please note that the shipping cost will be borne by the person.
It can be issued in municipalities in Address as of January 1 of that year.
(Example: If you have a Address in Yokohama as of January 1, 2024, you can obtain a municipal tax tax exemption certificate for fiscal 2024 at your ward office Tax Division.)
If you are unable to issue a tax-exempt (tax-exempt) certificate due to undeclared, you must first file a tax return.
Please ask the relevant municipal Tax Division for information on how to obtain the certificate and how to file it if it has not been declared.
Self-plans can be created in any format as long as necessary items are described.
