[Change of disease name indicated on recipient ID]
Based on the notification issued by the Ministry of Health, Labour and Welfare that came into effect in April 2024, the names of diseases listed on the identification card of recipients of the specified medical expenses (designated incurable disease) issued by Yokohama City will be partially changed as follows. In addition, the recipient ID for which the disease name has not been changed can be used as before if it is within the validity period of the recipient ID.

From April 1, 2022, the designation of designated medical institutions listed on the Certificate of Recipients of Specified medical expenses (Designated incurable disease) issued by Yokohama City is governed by "Medical institutions designated by prefectures or ordinance-designated cities based on the Act on Medical Care for Patients in incurable disease." If you are a medical institution designated by a prefectural government or a designated city under the incurable disease Law, you can use a designated medical expenses (designated incurable disease) recipient ID. As a result of this change, procedures related to changes (additions, deletions) of designated medical institutions will no longer be required from April 1, 2022 (changes other than designated medical institutions such as name, Address, insurance card, etc.) will continue to apply and report.)

・If the co-payment limit management slip shows that the total amount of medical expenses (100%) exceeds 50,000 yen per month, a copy of the co-payment limit management slip (for 6 months)
・If there is a refund from the specified medical expenses invoice, a copy of the specified medical expenses invoice (if the original has been submitted, a copy of the receipt)
※What is the continuation of expensive incurable disease treatment?
This is a special case for those who are certified and receive the medical expenses grant.
If the maximum monthly co-payment is “General Income I (10,000 yen per month)”, “General Income II (20,000 yen per month)”, and “Higher Income I (30,000 yen per month)”, apply by application.
The maximum monthly co-payment limit is reduced.

It is not necessary for those who have medical care certificate of "Specific medical expenses (Designated incurable disease)" or "Chronic Pediatric Disease" in Yokohama City.
・If you are in Yokohama or out of Yokohama, please fill out the application form "Persons who have been certified by the specified medical expenses of incurable disease" and "Persons who have been certified by medical expenses for chronic childhood illness".
Change of destination (if you wish to mail to other than Address in resident certificate)
Documents such as specific medical expenses (designated incurable disease) Recipient ID and renewal of recipient ID will be sent to your home (Address on resident certificate).
・If you have been hospitalized for a long time and cannot return home.
・When we want you to send to family living together or separated family due to dementia or visual impairment.
It can be set by application when you want to send it to someone other than your home or yourself. (attached documents is not required.)
※Please note that once you set the destination, renewal guidance and recipient ID will always be sent there.
Please apply for change again to return to your home or to yourself.
No widow (husband) deduction applicable
From September 1, 2018, the tax lawmedical expenses (designated incurable disease) grant grants will be applied to unmarried single-parent families, and will determine the hierarchy of the maximum amount of self-pay (monthly basis) as defined in the tax law.
