Overview

Until now, when using the medical expenses subsidy program in Yokohama, it was necessary to present the “Medical Certificate” along with the “Health Insurance Card” at the counter of medical institutions, etc., but from April 10, 2025, the medical expenses subsidy will be available on a minor insurance card. At the same time, information on Medical Certificate will be available on the minor portal.
You need a minor insurance card to use it.
Some medical institutions do not support them, so please bring a paper Medical Certificate at the time of consultation (the conventional paper recipient ID will continue to be issued).

To medical institutions and insurance pharmacies

With the introduction of the system, medical institutions, etc. will reflect patient information on the receipt computer (hereinafter referred to as "receikon") without entering the public expenditure recipient number, reducing the hassle of input.
In order to introduce the system, it is necessary to repair the Rececon of medical institutions.
The government subsidized the cost of system renovation, but the reception in 2024 ended on February 1, 2025 (homepage of reference country (outside site)).
In the FY2024 supplementary budget of the Ministry of Health, Labour and Welfare, a budget with the same content in mind was included. We will inform you as soon as the details are decided.

Details of the System

1 Progress of introduction

As one of the government's efforts to promote healthcare DX, the Digital Agency launched PMH (Public Medical Hub) initiatives in FY2023.
This service is intended to share information on recipients of the medical expenses grant with local governments, medical institutions, etc.
When a municipality registers Medical Certificate information with PMH, it is possible to link the minor insurance card with Medical Certificate.
Based on these national initiatives, Yokohama City was selected as a leading local government in fiscal 2024 for the Subsidies for Medical Expenses for Pediatric Treatment system, the medical expenses subsidy program for single-parent families and the medical expenses subsidy program for severely disabled peopleMedical Certificate.
As a result, from April 10, 2025, if you receive a medical examination at a medical institution in the prefecture that supports PMH, you will be able to receive a medical examination only with a minor insurance card.

2 Benefits

By using PMH, it is possible to use My Number Card as Medical Certificate. This eliminates the need for patients to bring paper Medical Certificate to medical institutions in addition to their health insurance card.
In addition, medical institutions can reduce the administrative burden of inputting Medical Certificate information and checking the latest qualification information.

3 Future Trends

The government is promoting the switch to a minor insurance card to promote medical DX, and plans to integrate with Medical Certificate and patient's card Shinryo-ken nationwide from fiscal 2026 onward.

In addition to the Medical Certificate for infant, Welfare Medical Certificate, and Medical Certificate for severely disabled people, we plan to integrate information such as the national medical expenses subsidy program (rehabilitation medical care, nursing care, etc.) with minor insurance cards for fiscal 2026 and beyond.
