Coordinator for Yokohama-type medical care children and persons

In Yokohama City, in order for medical care children / persons and severely handicapped children / persons to comprehensively coordinate the support of medical care, welfare, education, etc. necessary for home living and enhance the use of services at related local organizations, In cooperation with the Medical Association, a "Yokohama-type medical care child / person coordinator" has been assigned.
Visiting nurses who have completed specialized training in medical care, welfare, education, etc. will serve as a link between medical care children and persons and their families, local supporters and related organizations, and provide services necessary for home life. We will support you to live with peace of mind by receiving appropriate services.
The Coordinator will be located at the home-visit nursing station in Tsurumi Ward, Minami Ward, Asahi Ward, Isogo Ward, Aoba Ward and Tsuzuki Ward  .
