Please confirm "disability welfare" which introduced welfare service and contents that child with a disability, person living in Yokohama-shi and the families can use. The contents of the description are kept to a minimum, so please ask each window for details such as individual systems.

"Yokohama-type medical care child / person coordinator" is a visiting nurse who has completed specialized training in medical care, welfare, education, etc. We will support you so that you can live with peace of mind by acting as a link with institutions and properly receiving services necessary for home life.
