About notification of loss of National Health Insurance qualification by mail

In principle, the delivery of the National Health Insurance qualification transfer is based on the next agency.
However, if you have newly enrolled in social insurance due to employment, etc., and it is difficult to come to the ward office Hours for the procedure for disqualification of National Health Insurance, you can also apply for the loss by mail.
Notification method
Please send required documents to your ward office Insurance and Pension Division.

※Please note that if the documents you send are incomplete, you will not be able to complete the procedure.

※We are not responsible for postal accidents, so we recommend that you send them by simple registered mail.
※In cases where multiple procedures for joining or withdrawing National Health Insurance must be performed at the same time due to delays in notification.

※Premiums may differ from the calculated month and the paid month.
After the procedure, insurance premiums may be allocated and charged in the fiscal year after the date of disqualification, or insurance premiums may be refunded.

※After joining the employee health insurance workplace or the National Health Insurance Association, you will not be able to receive a medical examination at Yokohama City National Health Insurance. If you receive a medical examination, you may be charged the medical expenses paid by Yokohama City. For details, please refer to the refund of National Health Insurance benefits.
