Preparation of application (Please read first)
With regard to the participation of National Health Insurance in connection with the withdrawal of social insurance (employee health insurance), online applications using smartphones and personal computers are now possible in addition to conventional window applications and mail applications.

Prepare a My Number card and check the password (4 digits) of the digital certificate for user certification and the password for the digital certificate for signature (6 to 16 digits).

National Health Insurance Enrollment Procedures
This is a procedure for those who quit the employee health insurance workplace or the National Health Insurance Association to join the National Health Insurance.

New National Health Insurance subscribers or those who have a My Number Card at proxy/agent in same household.
（２） Flow after Online Application
After accepting the online application, the person in charge of the ward office Insurance and Pension Division will apply for National Health Insurance. After completing the procedure, we will send a “Qualification Confirmation Form” or “Notice of Qualification Information” to the head of the household.
After applying online, it will take about 1-2 weeks for the qualification confirmation etc. to arrive.
If you apply before the date of disqualification in the previous employee health insurance, it may take some time to complete the procedure after disqualification.

About minor portal
Online application has an electronic signature (public personal authentication), so an applicant's My Number card is required. Please check your My Number Card password before you start online application. Also, you need to install a minor portal app.
In this application, only the subscriber or the individual and proxy/agent of same household can apply online.

◎Digital certificate for signature (6 to 16 digits with mixed alphanumeric numbers)……Used when creating and sending electronic documents on the Internet, etc. (eg electronic application such as e-Tax). ◎Digital certificate for signature (6 to 16 digits with mixed alphanumeric numbers)……It will expire if you change Address or your name. If you want to continue using the service, you will need to change the information on the front of your My Number Card and reissue the digital certificate at the Registration Section office of your ward office in Family Registry Division.
In addition, you can reissue an electronic certificate at the My Number Card Special Center under the jurisdiction of your ward only if you have completed the procedure for changing the items described.
