About application for grant of "qualification confirmation" pertaining to person requiring consideration

Those who have difficulty in consulting with a My Number Card, such as a third party such as an assistant needs to accompany the insured person who is an elderly person or a disabled person to assist in confirming the qualification of the person.

You can submit a report to the Insurance and Pension Division National Health Insurance Section office of your ward office or by mail.

• Application for the issuance of a certificate of medical qualification for the latter-stage elderly and optional entry (Excel: 18KB) Example (PDF: 171KB)
• Copy of identity verification document (if someone other than the person or a relative of same household applies, the identity verification document of the delegated person)
• Power of Attorney (PDF: 190KB) Example (PDF: 342KB) (in the case of adult guardians, a copy of the certificate of entry, etc.)

・Documents with a face photo issued by a public office such as a driver's license, my number card, passport, etc.

Those that require confirmation of 2 points
・Documents without a photo issued by a public office, such as a health insurance card, a long-term care insurance card, or a pension notebook.
