1. In response to the formulation of the 2nd Yokohama City Community Welfare and Health Plan, the 2nd Midori Ward Community Welfare and Health Plan (2nd Midorinowa Support Ai Plan) will be formulated as an integrated plan with the Midori Ward Japan National Council of Social Welfare's community welfare activity plan.
2. The second plan consists of a ward plan based on the reflection and evaluation of the first plan, whose plan period ends in 2010, and a district-specific plan formulated in 11 districts in the ward.
3. About plan according to district, we establish "district support team" composed of each staff of ward, ward Shakyo, community care plaza and devise every each district.
4. The formulation period of the second plan is March 2011, and the plan period is five years from 2011 to 2015.
