Sample Emergency Medical Information Kit

The Emergency Medical Information Kit is distributed free of charge to elderly and disabled people living in Minami Ward to fill out and store medical information and emergency contact number in case of emergency such as illness or disaster. It is distributed at the Minami Ward Office and the Community Care Plaza.

Those who live in Minami Ward and fall under any of the following

  • Single and elderly couple households aged 65 and over
  • Persons with disabilities
  • Others (Japan and China solitary persons, etc.)

※There is one distribution per household.

[How to use the Emergency Medical Information Kit]

The emergency information sheet filled with medical information and emergency contact number is placed in a cylindrical plastic container and stored in the refrigerator. By attaching the seal of the mark to the "inside of the entrance door" and the "refrigerator door", you can prepare for emergencies.
